Included in this listing are the pharmacy average wholesale prices of each single unit of medication. This is used as the base to calculate the final patient charge which will vary based on the average wholesale cost plus a markup.

Your own charges and out-of-pocket expenses will depend on the actual patient care services you receive, the terms of your insurance coverage, and/or your eligibility for financial assistance.
